Staff New Hire Forms
You may download and complete the following forms prior to your first day of work. Proper identification for employment verification (see I-9 for list of acceptable documents) is required. Please call Human Resources at 615-460-6456 to confirm your appointment for new employee processing.
Annuity Participation Form
As a full or part-time employee you are entitled to tax-shelter part of your salary. This form is only acknowledgement that you understand your rights to participate and in no way obligates you to participate.
Direct Deposit
The direct deposit form authorizes Belmont to make direct payroll deposits into your checking or savings account. You can designate funds in up to 4 separate accounts.
Employment Information Sheet
The employment information sheet is a collection of emergency contact information that is maintained in the Office of Human Resources and entered into our HRIS system.
I-9
This is a federally mandated form that verifies identity and employment eligibility. Be sure to bring documentation with you to the Office of Human Resources when you submit your paperwork. A list of acceptable documents is located on page 4 of this form.
Parking/Identification Form
This form authorizes Campus Security to grant you a parking decal and employment photo identification card. All paperwork must be completed through Human Resources prior to issuance of an ID card.
Voluntary Veteran Status Form
The Veteran status form is a voluntary disclosure to certify if you were/are a veteran.
W-4
The W-4 allows you to specify with-holdings of federal income tax from your paycheck(s). Please complete (lines 1 ,2,3,4,7 if you are claiming exempt status) and sign the form to validate it. According to the IRS you cannot claim exemption from withholdings if your income exceeds $850.
Tutorial Acknowledgement Form
This form acknowledges that you will complete the online tutorials for FERPA and Sexual Harassment.
Workshop Confirmation Form
The Workshop authorization form confirms new hire participation in various levels of mandatory workshops which could include but are not limited to New Staff Orientation, Discovering Yourself at Belmont , Taking Care of Business as well compliance with online completion of FERPA and Sexual Harassment Awareness tutorials. You should complete the form by listing your name, department, phone and date. The Office of Human Resources will enroll you in available and appropriate work related classes.
